Okay on my second watch of Reichenbach I viewed this scene a little differently. Sherlock has just had the realization he’s going to die, or at least that that’s Moriarty’s ultimate plan, and his first thought is… Molly? The first watch I brushed the feeling of oddness off because a) I didn’t know what Sherlock was doing so I assumed it would maybe be explained and b) I was emotionally compromised and couldn’t think thoughts beyond “no” and “make it stop”, but on the rewatch I knew the ending, and I knew that after this we don’t actually see Molly again, or really anything to do with her line of work, so why would he need her? And then I remembered, oh, yeah, the only dead body we see after this (besides Moriarty’s) is Sherlock’s. Except it’s not really Sherlock’s, is it? So, where would he have acquired a body?
tl;dr Molly knows Sherlock’s alive because she helped him fake his death.
